Description (Service)
  • Provides assistance to clients upon hospital discharge 
  • Partners with the Canadian Red Cross to offer services
Services can include any or all of the following: 
  • Travel with patient from the hospital/medical facility to home 
  • Prescription and medical supplies/equipment pick-up 
  • Grocery shopping 
  • In-home visual safety check 
  • Booking follow-up appointments/community support services 
  • Settling-in assistance such as light housekeeping, laundry, ordering Meals On Wheels etc

Service pre-determined for time of hospital discharge
Areas Served Englehart ; Kearns ; Kirkland Lake ; Larder Lake ; Matachewan ; Virginiatown
Eligibility Patients 55 years and older who: 
  • Are being discharged from hospital and require transportation and/or settling-in assistance 
  • Are able to direct their own care 
  • Are medically stable and ambulatory 
  • Are able to be left unattended 
  • Do not live in long-term care or facility with 24-hour care 
  • Are either admitted on inpatient units or have presented at the Emergency Department
Application Referral is made by the hospital discharge staff and sent to the Client Services Coordinator for arrangements
