Description (Service) A specialized outpatient program to support individuals with wounds that are slow to heal
 
  • Supports the physical, mental, emotional, environmental, nutritional and financial needs of the individual 
  • Provides consultation with other healthcare providers as needed 
  • Provides Telemedicine appointments as appropriate 
  • Offers a multidisciplinary team consisting of Enterostomal Therapy Nurse, Physiotherapist, Occupational Therapist, Dietitian, Social Worker, Chiropodist, and Physician
Hours Mon, Thu 8 am-6 pm * Tue-Wed, Fri 8 am-4 pm

Eligibility Individuals with: 
  • A wound that has not decreased in size by 40% within four weeks of being treated 
  • A chronic disease as a risk factor, such as Diabetes, peripheral vascular disease, arthritis, neurological disease 
  • Weakness and/or limited mobility 
  • Poor nutritional intake and poor appetite 
  • Obesity or are underweight 
  • Difficulties performing self-care tasks such as hygiene
Application Referrals can be made by the client, family, caregiver or healthcare professionals
