Description (Service)
  • Provides a comprehensive geriatric assessment via telemedicine (using videoconferencing and other electronic equipment) with a focus on identifying resources and strengths of the client 
  • Community services are identified and/or initiated to keep clients in their home and community and a coordinated care plan is delivered based on the assessment needs of the client and their families
Hours Office Mon-Fri 8 am-5 pm
Areas Served Kenora District ; Rainy River District ; Thunder Bay District ; NWLHIN 14, not including the city of Thunder Bay
Boundaries Clients living in the city of Thunder Bay can access Geriatric services at St Joseph's Hospital
Eligibility Ages: 60 year(s) and up 
Seniors 60 years and older or physiologically and/or cognitively equivalent who have: 
  • deteriorating health or functional status 
  • safety concerns 
  • frequent or increased use of health services 
  • altered quality of life of support services
Application Physician/Nurse Practitioner referral is required, though clients without family doctors can access referrals through walk in clinics
