Description (Service) Medical care team focused on family and community health 
  • Offers timely access to high quality primary care 
  • Provides access to other health care professionals within and outside of the Family Health Team 
  • Coordinates documentation if patient receives care from a member of the team 
  • Participates in teaching with a regular rotation of medical students and residents from Northern Ontario School of Medicine (NOSM) 
Offers primary health care including but not limited to: 
  • Chronic disease management 
  • Disease prevention 
  • Health promotion 
  • Mental health counselling 
  • Nutritional counselling 
  • Palliative care 
  • Rehabilitation
Diabetes Educator/Services 
  • Coordinates educational group sessions for diabetes self-management 
  • Provides designated Diabetes Days to screen and review patient's progress and to provide an opportunity to discuss personal concerns with nurse practitioner, diabetes educator and doctor
Hours Mon 8 am-7 pm, Tue 9 am-5 pm * Wed-Thu 8:30 am-5 pm * Fri 8:30 am-4:30 pm

Eligibility Patient must be rostered with Family Health Team to access services; form available at front desk
Application Appointments required, no walk-ins; but same day appointments may be available
