Description (Service) Medical care team focused on family and community health 
  • Offers timely access to primary care  
  • Provides access to other health care professionals  
  • Coordinates documentation if patient receives care from a member of the team 
  • Provides a sexual health clinic that includes testing and treatment for sexually transmitted infections, contraception health, and prenatal and postnatal care 
  • Provides chronic disease prevention and management services
Health care team includes: 
  • Diabetes Educators 
  • Dieticians 
  • Nurse Practitioners 
  • Physicians 
  • Registered Practical Nurses 
  • Social Workers 
  • Wound care nurse
Hours Mon-Fri 8:30 am - 4:30 pm
Areas Served Thunder Bay
  • Patients of Anishnawbe Mushkiki who who need immediate medical care for minor illnesses and injuries  
  • Must have a valid Ontario Health Insurance Plan (OHIP) card
