Description (Service) Specialized case managers and rehabilitation professionals work directly with family members, friends, other service providers and clients living with the effects of an acquired brain injury to improve: 
  • Daily living skills 
  • Communication 
  • Community orientation and integration 
  • Emotional support 
  • Recreational opportunities
Services offered include: 
  • Outreach Rehabilitation Services 
  • Housing Programs 
  • Community Support Groups

  • Individuals living with the effects of an acquired brain injury 
  • Must have a documented acquired brain injury for admission to housing services 
  • Must have a valid Ontario health card 
  • May not be in receipt of insurance funds
