Description (Service)
  • Provides case management services to adults with mental illness, concurrent disorders and developmental challenges 
  • Services are designed to promote recovery and independence while providing appropriate community based support
Provides the following services: 
  • Assistance in accessing mental health and/or psychiatric services 
  • Help to integrate into the community, advocating with informal and formal community resources 
  • Referrals to community agencies, where appropriate 
  • Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program 
  • Housing rent supplement 
  • Assistance in obtaining and maintaining housing in the community
Hours Mon-Fri 8:30 am-4:30 pm

Eligibility Ages: 16 year(s) and up 
  • Must have a diagnosis or display symptoms of a mental illness and developmental delay 
  • Resident of Thunder Bay 
  • Must require assistance and support in order to live successfully in the community
