  • Provides case management services to youth age 16 to 24 who have either a confirmed diagnosis of mental illness or symptoms of mental illness 
  • Offers services designed to promote the skills necessary for youth to live independently in the community while coping with the symptoms of mental illness
Hours Mon-Fri 8:30 am-4:30 pm

Must have a confirmed diagnosis of mental illness or be experiencing symptoms of a mental illness
Application Clients can self-refer to program, be referred by a third party (family member, health care provider, etc) or apply for service through the Northwest Access Point
