Description (Service)
  • Helps individuals to participate in meaningful life roles at, or as close as possible to the level they did prior to their acquired brain injury  
  • Provides flexible services to meet the diverse needs of individuals who require rehabilitation and support services  
  • Provides referrals to the residential services available in Thunder Bay
  • Offers Assisted Living Services in the following locations: 
    • Castlegreen Site 
    • Cumberland Place 
    • McKellar Place
  • Provides Community Rehabilitation and Outreach Services in Thunder Bay, Kenora, Fort Frances and Dryden
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Algoma District ; Cochrane District ; Kenora District ; Manitoulin District ; Nipissing District ; Rainy River District ; Sudbury District ; Thunder Bay District ; Timiskaming District ; All services are provided out of Thunder Bay, Dryden, Fort Frances, Kenora and Sioux Lookout. Some services are delivered through Ontario Telemedicine Network (OTN) and other technology to deal with travel and climate
Eligibility Ages: 16 year(s) and up 
Individuals with an acquired brain injury
  • Referrals by family, health professional, physician, lawyer, or self-referral
  • Intake process includes completed and signed "Application for Serviceā€ (AFS) form which can be found on the website 
  • AFS form can be signed by substitute decision maker
