Description (Service) Provides services that promote physical and mental health for community members 
  • Coordinates health care services as a department of the band office  
  • Facilitates consultations through visiting professionals such as a physician, nurse practitioner, occupational therapist, physiotherapist, registered dietitian, chiropodist and a mental health counsellor 
  • Provides health awareness and counselling, education and prevention workshops especially in the areas of diabetes and alcohol/drug abuse 
  • Coordinates clinics/workshops in areas that may include Diabetes, Environmental Health, Immunization, Pre/Post Natal Health, Sexual Health and Communicable Diseases
Addresses individual health care needs by providing:  
  • Advocacy on behalf of individual with other health professionals  
  • Coordination of one-on-one pre-natal care with the community health nurse  
  • Home visits to the sick, elderly, new mothers and persons with health care needs 
  • Assistance with coordinating local and long distance travel arrangements for medical appointments
Coordinates the following health programs: 
  • Family Home Visitor 
  • Health Promotion 
  • Long Term Care 
  • Long Term Care, Handi-Trans Bus 
  • Medical Transportation
Coordinates the following social services programs: 
  • Community Support Services 
  • Community Wellness 
  • Food Bank 
  • Parent Resource Coordinator 
  • Youth Program
Ontario Telemedicine Network (OTN) 
  • Provides access to specialized medical care using videoconferencing and other tele-diagnostic equipment 
  • Allows specialists to remotely examine and prescribe treatments to patients in their home communities 
  • Reduces the need to travel to receive care
Hours Office: Mon-Thu 9 am-4:30 pm, Fri 9 am-4 pm 
Clinic Days: Mon, Wed-Thu 8:30 am-2:30 pm * third Thu of each month 1:15 pm-6 pm

  • Clinic Days: booked appointments with Nurse Practitioner recommended but will accept walk-ins
