Description & Services
Description (Service)
  • Focuses on reducing the impact of poverty for those in the greatest need and identifies gaps and duplication in services 
  • Assists with medical transportation through the Northern Health Travel Grant (NHTG) 
  • Assists individuals with one time emergency funding for a direct service 
  • Provides referrals to appropriate community services to assist individuals with their personal needs
 
Provides financial assistance in the following areas: 
  • Eyeglasses 
  • Baby safety items (crib, crib mattress, car seat, baby gate, prescriptions for children that otherwise may not be covered by OHIP+) 
  • Emergency dental services 
  • Rental arrears (to avoid eviction) 
  • Utilities (to avoid disconnection)
 
Northern Health Travel Grant 
  • Assists individuals who are eligible for the grant by advancing money for the travel costs
Hours Mon-Fri 9 am-12 noon, 1 pm-4 pm
Areas Served Algoma District
Eligibility Ages: 18 year(s) and up 
 
  • Low to moderate income households 
  • Individuals who received a threat of eviction (an N4 form -- Notice to End Tenancy Early) for rental arrears
 
Northern Health Travel Grant assistance 
  • Individuals who must travel long distances to receive medical care or services not available locally, but cannot afford to pay the costs of travel and accommodation that are eligible for reimbursement from the program
Application
  • Initial intake by phone 
  • Must provide documents to support requests 
  • Identification required 
  • Rent or Utility Arrears assistance: applicant must complete a Housing Stability Bank application 
  • Northern Health Travel Grant assistance applicants: 
    • Must present paper copy of grant application filled out by local doctor 
    • Must reside in the Algoma District 
    • Must not be receiving benefits from Ontario Works or Ontario Disability Support Program 
    • Must meet standards of income assessment
