Description (Service) Delivers medical transportation services and assistance in making medical appointment arrangements 
  • Provides transportation from/to Seine River Reserve to any medical appointments (doctor, dentist, optometrist and mental health counselling) in the town of Fort Frances 
  • Provides assistance with making arrangements for any medical appointments out of town such as booking appointments
Hours Health Office: Mon-Fri 8:30 am-4:30 pm

  • Requires 24 hour advance booking; exceptions made for emergencies 
  • Must have medical trip authorization form signed by doctor/health professional during appointment and return it to Health Services Office  
  • Advance cancellation recommended
