Description & Services
Description (Service) Supports diabetes management clinics in Treaty 3 communities to assist individuals in understanding and managing their diabetes 
  • Provides diabetes resources and educational tools to communities  
  • Offers education and self-management support to fit an individual's lifestyle  
  • Provides access to Registered Nurses and Registered Dietitians 
  • Maintains a library including many diabetes resources available upon request
Diabetes prevention and management tools and resources available to all Treaty 3 communities include: 
  • Diabetes Medicine Bag (DMB), an educational program for health educators designed to help prevent type 2 diabetes among Aboriginal children and youth 
  • Diabeters Bags, with tools for children, youth and adults to teach healthy eating and portion control 
  • Diabetic Foot, which shows common foot problems for diabetics and demonstrates how diabetes impacts glucose in the bloodstream 
  • Insulin Resistance Globes, A1c Sticks, portion plates and other teaching tools
