Description (Service) Provides supportive housing and personal support to enable individuals to live independently in their own apartment 
  • Promotes mental and physical health 
  • Provides activities and outings for program members
Offers assistance from an on-site support worker with maintaining and developing daily living skills including:  
  • Budgeting and personal money management 
  • Grocery shopping 
  • Health care coordination  
  • Healthy meal planning and preparation  
  • Housekeeping 
  • Medication reminders  
  • Personal care
