Description (Service)
  • Provides a diabetes education program to help individuals make informed decisions about diabetes care and management 
  • Works in collaboration with the physicians at the Chapleau and District Family Health Team 
  • Provides a weekly drop-in visit for blood pressure checks, blood sugar checks, weigh-ins and healthy recipe ideas
Offers the following services: 
  • Blood sugar and blood pressure checks 
  • Education on diabetes 
  • Nutrition counselling specific to diabetes, high cholesterol, and high blood pressure 
  • Ordering of blood work 
  • Diabetic sensory foot screening 
  • Referrals to supports including chiropody and CNIB
Hours Mon-Thu 8:30 am-4:30 pm 
Weekly Drop-in Tue 2 pm-3 pm
