Description (Agency) Community Living Ontario advocates for, promotes and facilitates the full participation, inclusion and citizenship of people who have an intellectual and/or developmental disability
Agency Services
Description (Service)
  • Provides services and supports to meet the needs of adults with an intellectual and/or developmental disability 
  • Works on a "person-centered" model to address the values, priorities and preferences of each individual 
  • Administers a residential group home in the community which provides 24-hour support and supervision
Person Directed Planning (PDP) 
  • Identifies what individuals needs and wants are in relation to life in general (assistance from family requested)  
  • Identifies individual's personal, educational, vocational, medical, and safety support needs in relation to wants  
  • Connects and supports individuals in accessing resources to meet identified needs and wants of the individual
Provides the following programs and services to enhance individual growth and independence: 
  • Day Program 
  • Passport Program 
  • Respite Program 
  • Supported Employment Program 
  • Supported Independent Living
