Description (Service) Provides case management services to individuals and families struggling with mental health and addictions issues
Case Management Services include the following: 
  • Assessment 
  • Counselling 
  • Crisis intervention and support 
  • Client advocacy and service coordination 
  • Family support and education 
  • Medication monitoring 
  • Skills development
Hours Office Mon-Fri 8:30 am-5 pm 
Community Mental Health Sites: Each one open Mon-Fri 8:30 am-5 pm
Areas Served Attawapiskat First Nation ; Fort Albany First Nation ; Kashechewan First Nation ; Moose Factory ; Moosonee ; Weenusk First Nation - Peawanuck
Eligibility Ages: 16 year(s) and up 
First Nation members and residents of the Weeneebayko region struggling with mental health and addictions issues

  • Referrals accepted from any source
