Description (Service)
  • Provides support services to help individuals living with a psychotic disorder or other diagnoses to live in the community  
  • Supports individuals who have experienced frequent or lengthy hospital stays or who have used mental health services, but have not benefited from these services  
  • Offers services in community locations on an on-going basis
Offers a multidisciplinary team of health care professionals, including:  
  • Nurses  
  • Occupational Therapists  
  • Peer Specialists  
  • Psychiatrist  
  • Social Workers
Services offered to identified clients can include: 
  • 24-hour crisis support 
  • Advocacy 
  • Assistance with activities of daily living 
  • Family-centred services  
  • Medication management  
  • Primary care services  
  • Social and leisure-time support  
  • Substance abuse services  
  • Symptom treatment, including regular visits with a psychiatrist  
  • Work-related services

Mon-Sun 24 hours
Areas Served Cochrane District ; Timiskaming District
Eligibility Ages: 16 year(s) and up 
  • Individuals who frequent hospitals for treatment related to a serious mental illness (ie main diagnosis of schizophrenia or bipolar disorder)  
  • Examples of eligible individuals can include those who do not take medication as required or who have not benefited from traditional services
  • Intake assessment required  
  • Approval for services is determined through collaboration between treatment staff, referring source and applicant
