  • Centre de santé communautaire du Grand Sudbury 
  • Elizabeth Fry Northeastern Ontario 
  • John Howard Society of Sudbury 
  • N'Swakamok Native Friendship Centre 
  • Sudbury Action Centre for Youth

Description (Service)
  • A collaborative partnership of six agencies dedicated to help end chronic homelessness and assist those who experience homelessness find and maintain housing 
  • Offers case management, street and shelter outreach and housing navigator services
Hours Mon-Fri 8 am-3:30 pm
Areas Served Greater Sudbury
Eligibility Ages: 16 year(s) and up 
Individuals must be able to, and willing to, participate in a program that consists of weekly home visits by their assigned case manager, once they find housing
