Description (Service) Provides various health benefits to individuals and families with high health costs
 
Benefits are determined based on need and may include: 
  • basic dental care 
  • diabetic supplies 
  • eyeglasses  
  • hearing aids  
  • prescription drugs  
  • surgical supplies and dressings  
  • transportation for medical appointments
Hours Mon-Thu 8:30 am-12 noon, 1 pm-4:30 pm * Fri 8:30 am-12 noon, 1 pm-4 pm

Eligibility Members of Dokis First Nation 
 
Individuals and families who:  
  • were eligible for Ontario Works in the previous months, or  
  • are no longer eligible for Ontario Works because they received a payment for loss of income under the 1986-1990 Hepatitis C Settlement Agreement
Application Individuals can apply by contacting the Ontario Works Administrator
