Description (Service) Administers a functional assessment once an individual has been accepted for assistance
Develops customized assessments that provide information on: 
  • An individuals attendant care needs 
  • What community resources are available 
  • Training required to help achieve goals
Assessments provide an analysis of abilities in the following areas: 
  • Community Integration Skills 
  • Cognitive Training Skills, including attention, perception, memory and learning, organizing processes, reasoning, and problem solving and judgment 
  • Communication Skills 
  • Directing Personal Care Needs 
  • Exercise/Fitness Programs 
  • Healthy Eating/Lifestyles 
  • Housekeeping 
  • Life Skills 
  • Managing Time, Goals, and Stress 
  • Mobility 
  • Money Management 
  • Safe Living Skills 
  • Self Development Skills 
  • Upper extremities
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Greater Sudbury
Eligibility Ages: 16 year(s) and up 
Individuals must: 
  • Have a valid OHIP card 
  • Require physical assistance due to a permanent disability 
  • Be medically stable 
  • Be capable or potentially capable of directing their services 
  • Be able to live safely unattended (assistance is by appointment only)
