Description & Services
Description (Service) Connects individuals with a Stroke Community Navigator
Helps individuals with: 
  • Understanding the health care system, what services are available to them, and what services are best for them. 
  • Finding transportation, in-home nursing, personal care/housekeeping, physical activity programs 
  • Coordinating with agencies to help with home modifications or permanent housing 
  • Accompanies individuals to appointments, if required
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Greater Sudbury
Eligibility Ages: 16 year(s) and up 
Individuals who have suffered from a stroke
Individuals must: 
  • Have a valid OHIP card 
  • Require physical assistance due to a permanent disability 
  • Be medically stable 
  • Be capable or potentially capable of directing their services 
  • Be able to live safely unattended (assistance is by appointment only)
  • Call for more information 
  • The application form is available on the website
