Description (Service) A Housing First Program where workers provide outreach services to individuals/families assessed and determined to be chronically homeless < /br> 
Supports Include: 
  • Assistance in finding and maintaining housing, by reinforcing tenancy requirements such as payment of rent and utilities, and learning the skills necessary to maintain permanent housing 
  • Orientation to an individuals surroundings and neighbourhood 
  • Developing individual service plans to identify and address barriers 
  • Assistance with accessing long term community services and supports
Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm
